If it happens to fall on Mon-Fri, I'm usually at my desk. Every now and then I may take off.

It depends, this year my birthday fell on a thursday, and I'm normally off thursdays.
Last year it was on a Wednesday, since my Wednesday shift is overnight, I requested it off.

Birthdays and holidays are really like every other day, where I work. Hospitals never close, so there's nothing special about one day as opposed to another, except for holiday pay on those particular days.

I always take the day off. I can only think of one time when I worked on my birthday, and that was when I turned 21 (ironic that that was the one).

I try my best to avoid working on my birthday. This year I was lucky enough to have that day off anyway, but someone tried their best to get me to come in and work for them on it. I lol'd. There was no way that was going to happen!
