Do you think it's inappropriate to date (or get intimately involved with) your friends ex? Have you ever?

No, I never have. As a blanket statement, no I don't think it's inappropriate but it really depends on the situation. How good a friend are they? Do they mind? Are you going to have to choose between your friend or their ex?, etc.

It's inappropriate, but I've done it...
My friend claimed he didn't care, but I think he did.

I don't think it is wrong but anyone who does it should be prepared to lose the friendship or at least have the dynamic of the friendship change.

Not sure there's a yes or no answer to this. It depends on so many things. How close that friend is to you. If they're ok with it then obviously there's no problem. But if they're not then you have to decide what's more important, the friendship or the potential relationship. If it were a very close friend & it bothered them, then no. Their friendship means too much to me.

Re: Dating your friends ex
It all just really depends. I've done it before, when the guy was much more into my friend than she had been into him so she didn't care. It sucked though, because I kept comparing his feelings for me to his feelings for her and they didn't measure up. He'd worked so hard to woo her and didn't have to do that with me. Made me feel bad about myself.
I later WANTED to date an ex of hers that she had been madly in love with. I didn't do it though.
