Several times as a child, once because I was on the edge and the sheet snapped out of the corner on the other side. Not as an adult though, but I do have plenty of those damn falling dreams where I can actually feel the ground disappear under my feet, wake up in bed and still nothing under my feet but the mattress under my back is a big relief.

Not on purpose. My father told me that when I was still in a crib he came into my room and I'm standing in the crib but the matress was on the floor.

I almost fell out of bed a couple of times, I guess I was tossing during the night and ended up on the edge of the bed. When I woke up I saw how close I was and moved over.

Not fell but once when I was REALLY sick - my Mom figures it was the Norwalk Virus - I stood to get out of bed and fainted - woke up a a few seconds later and was on the floor but other than that, I don't think I've fallen out of bed unless it was when I was little.

Yeah I have. It's weird to fall out the bed when you sleep....it almost feel like you're falling off the earth.

Not that I know of, but maybe as a kid.
I always think about how so many kids have bunk beds, and yet I never hear stories of them falling out. But yet parents tell stories of kids falling out of regular beds. I know that newer bunks have rails, but the old school ones do not.
