Well, guess I'll be the lone voice here to say that if there were no objections to having sex outside of the relationship then I probably would. (In fact I did).
Sex can be gratuitous without a relationship, and a relationship is more than sex. The two need not be exclusive. Or so it has been in my experience.

I had a really sick ex who had a muscle disorder. Whenever we had sex, his muscles would all tense up and cramp... he would be in pain for hours, sometimes he would vomit. He would swear the sex was worth the pain, but more often than not, I would wind up crying afterwards watching him writhe in pain.
Because of this, sex was almost non existent. And yeah, I was loyal to him. but if this arrangement were proposed, i probably would have acted on it. everyone has needs
