How relevant to my current life! lol (Well, kinda) Yes, we complain usually. lol We're paying for it so it's not as if we're not right to complain. The good restaurants will gladly fix it for you.

It just depends on where they screwed up. If I can live with it or it's really not a big deal sometimes I let it pass. Other times I want what I ordered.

If something is seriously wrong then I will definitely send it back. But I will not take a replacement or re-order. I don't trust restaurant cooks when it comes to this.
The other day I left a plate of seafood pasta mostly uneaten, and when the waitress asked about it I told her that I didn't like it because it tasted way too "fishy". It was microwaved, which a restaurant should NEVER do with any kind of seafood.

It depends on the error. We went out to eat for Valentine's Day and the table next to us had problems. The one girl ordered a personal size pizza and was given a calzone. Then the other girl bit into her panini and it had meat in it and she ordered no meat. I was really expecting the place to offer them free dessert or free meal, but they did not.

IDK if she was just not into meat in that instance or if she was a vegetarian. I woulda made a noticeable exit if someone served me meat. I make it clear that it would not be Kosher.

It seemed more to me it was a preference. The girls were kind of chatting about the pizza/calzone mix up when she took her first bite, then said, "This has meat in it." She seemed more amused than disgusted. So, she got another one.

If the screw up is in my favor, then I won't say anything. I tend to be oblivious with stuff like this though and will probably eat 1/2 of it before I notice that they gave me the wrong food.
