I agree. It's not that easy to pinpoint when hitting someone is appropriate. It's one of those impromptu situation where the necessity of it becomes obvious in the moment.

If someone delivers the first blow then I think its appropriate to hit them back.

It depends, but yes I do think there are circumstances where delivering the first blow is appropriate.

I agree that it's a case by case situation. Just because someone insults me isn't enough. They may try to hit me because of my verbal reply though and then it's Katy bar the door!
