When I first started signing my name I did but not any more. Which isn't easy to get away from. Sometimes you get stuck with a sig because it has to be consistent like on income tax forms and licenses.
I finally got it changed on my Drivers' License so now it is considered my official sig - but IRS and Social Security still have my original one with MA from the early '60s.

My college had a strict rule of forms and records being signed exactly with how I was registered. When I applied and eventually registered for classes I made the mistake of including my middle name even though it wasn't needed and as result I had to sign anything college record related with my middle name included.
