    I once reported a co-worker who hit a patient. He got fired. His hand print showed up on the guy's skin.

    I have complained to superiors about co-workers, but it was nothing formal.

    I wrote a 12 page letter documenting all of the mail that a supervisor purposely and incompetently delayed--- plus all of the comments that he had made which questioned our religion, criticized us for being working women, and the threats he made on a regular basis. His favorite comment to me was "I've delved too deeply for you to comprehend"!!! I HATED this line. WTF??? This was the most racist, sexist, arrogant person I ever met in my life.

    The second complaint was against a former bf who was stalking me, both in and out of work. The police did nothing, so the postal inspectors went after him.

    I've reported various people for various things, over the years, but we have no "formal" report system--it consists of tossing the supervisor an email the situation. Depending on the supervisor, resolution may or may not get done. . . .

    My most recent one was turning in someone who kept eating in the lab, no matter how many times he got written up for it. So I snapped a photo on my cell phone of his candy bag on the counter next to all the specimens, and emailed that to the supervisor. He no longer works with us, but left of his own accord after that.

    Being a supervisor, I've written people up....but I've never gone directly to HR and put in a formal complaint against anybody.
